This is very, very good for getting customer feedback, market research, or for a simple lead generation quiz. The user experience is just better, and you will get more responses.

The final question. Is MetaSurvey worth it? This is a very complex answer. You must listen carefully.
The tool itself is very good. The user interface is beautiful. The “swipe” idea is fantastic and it really works to get more responses. It is fun to use.
BUT. This is very, very important. You must look at the plan limits. The Metasurvey lifetime deal is, to be honest, very expensive. And even on the highest tiers, they only give you one custom domain. This is a huge problem. If you are an agency or you have more than one website, this is a big, big limitation. Many users are very unhappy about this. To get more custom domains, you have to pay a monthly fee. This is not a real lifetime deal.
So, my verdict is: the product is excellent. But the deal terms, especially for the custom domains, are not good. You must be very careful and decide if that one domain is enough for you.
